Contact Lens Training
$25.00
Contact lens insertion and removal training is required if you have never been prescribed contact lenses previously. A trained technician will sit with you and teach you how to put in, take out, and care for the lenses. The training fee will cover up to 3 separate training sessions.
Thumb Drives
$10.00
If you would like a copy of your exam, prescription and/or Retinal Imaging (OPTOS) results we will put them on a new USB thumb drive for your records! (provided by Perks of Eye Care). If you would like to provide your own USB thumb drive for no charge, It must be new in packaging!
Forms
$10.00-$20.00
Any forms brought in and required to be filled out by our Optometrist will result in a charge. This includes but is not limited to the following: ADOT, School vision screenings, specialty pharmacy requests, etc. If you need a ADOT form provided to you the charge can go up.
